A Practice Built on Generations of Trust
For over 60 years, Apple Hills Medical Associates (AHMA) has proudly served the Applewood community, delivering comprehensive, community-based primary care. Our physicians have supported generations of families — from delivering babies and making house calls, to providing hospital care and palliative support, and remaining an integral part of the Applewood community.
Primary care is often your first point of contact with a doctor or health care team. It includes a wide range of services such as disease prevention and management, treatment, rehabilitation, palliative care, and health promotion. Over the years, AHMA has been committed to delivering quality primary care — and that commitment has never wavered.

Since 2007 Apple Hills Medical Associates is now part of the Summerville Family Health Team. This partnership allows us to build on our longstanding commitment to your care by expanding access to a broader team of health professionals, all working together to better support your health.
What is a Family Health Team?
Family Health Teams were created by the Ontario Ministry of Health to enhance primary care by integrating physicians with other health care providers. This collaborative model ensures you receive the most effective, timely, and coordinated care possible.

At Apple Hills, you'll still have access to your family doctor during regular office hours and continue to benefit from our extended evening and weekend hours for urgent concerns. Now, as part of Summerville Family Health Team, you also have access to:

Support patient care, health education, and care coordination within the team.

Help patients navigate social determinants of health, mental health support, and community resources.

Provides nutritional counselling and dietary guidance for chronic disease management and overall wellness.

Health Promotion Specialist
Supports patients in making healthy lifestyle changes and navigating preventive care programs.

Reviews medications, supports adherence, and works with your physician to optimize your prescriptions.
These professionals work closely with your AHMA physician to support your ongoing care. Whether you see a doctor or another provider in the team, your personal AHMA physician will stay informed through shared documentation and electronic medical records.

As part of Summerville, we've introduced electronic medical records and enhanced digital communication with private labs and local hospitals. This means:

By working alongside other health care professionals, we can coordinate care in new ways and enhance the skills of each provider to ensure the best possible care for you, our AHMA patients.
After-Hours Support
Care That Doesn't Stop at 5 PM
Outside of office hours, you can still contact the Telephone Health Advisory Service, where a registered nurse will advise you on urgent health concerns. With your permission, a summary of your call will be sent to your AHMA doctor the next day to ensure continuity of care.
